🔟 Consider alternative diagnoses
If villus atrophy but TTG IgA not checked → confirm first.
DDx: tropical sprue, CVID, HIV enteropathy, giardiasis.
Don’t commit to lifelong gluten-free diet without serology.
9️⃣ Collaborate with pathology
Borderline results? Review slides with GI pathologist.
They can clarify orientation, suspicion, & alternative diagnoses.
6️⃣ Don’t forget the stomach
Duodenal intraepithelial lymphocytosis (Marsh 1) is nonspecific. Gastric biopsies help distinguish early celiac vs mimickers (e.g., H. pylori). Lymphocytic gastritis may also coexist in celiac.
4️⃣ Take enough biopsies
Submit 4–6 pieces, including duodenal bulb.
Up to 10% of new cases limited to bulb (“ultra-short” celiac). Missing bulb = missed dx.
3️⃣ Biopsy even if duodenum looks normal
Celiac can present w/ anemia, wt loss, abd pain.
Endoscopic findings often subtle.
Don’t skip biopsy if suspicion or serology unclear.
2️⃣ Elevated TTG IgA?
Do an EGD w/ duodenal biopsy.
10x TTG IgA + Endomysial antibody may predict celiac, but biopsy is still the gold standard before prescribing lifelong gluten-free diet.
1️⃣ Order the right serologies
TTG IgA = most sensitive/specific
Always check total IgA (to catch IgA deficiency)
If IgA deficient → use IgG (TTG/DGP), but specificity lower
HLA DQ2/DQ8 testing = high NPV, not for routine dx
