Which ear had the cochlear implant?! This is a postop audiogram prior to activation. Gray is the preoperative. Shared with permission. Crazy that this is a regular occurrence for me. I counsel patients, "It's possible but unlikely that you may lose your residual hearing"

Stabilizing any human, legal or not, in an ER. is not healthcare. It’s triage, till they can kick them back onto the street. What’s fucked up is that neither party wants Healthcare. The Dems want to send more money to insurance companies rather than dealing with the problem of helping patients pay for care. The Reps want to rage farm about illegals for the sole purpose of creating content that the algorithms and their podcasters love. . Zero consideration for what will happen if any human is not able to get stabilized in an ER, and remains sick and/or injured in public. Have the OMB analyze that.

CBO projects the population to barely grow over 30 years (and decline outside of immigration). The under-65 population will begin declining next year. We can't sustain 3% economic growth or pay for Social Security & Medicare without healthy workforce growth. This will get ugly.

I said I wanted to fix healthcare in the USA. That’s where I’m spending my money. Some people think helping Americans get out of this healthcare mess we are in is important. In fact I think it’s the most important issue Americans face. Some don’t. They want to talk about things I can’t impact, I want to talk about things I can change. Healthcare will cost the typical American Family of 5 , 5x what they pay in federal income taxes. BEFORE deductibles and max out of pocket It will cost your employer 30k for that same family. Leaving a lot Less money to pay to you. And if you have a high deductible and high out of pocket plan, you are just fucked unless you are rich. Really tough when most people who don’t qualify for medicaid barely have $500 in the bank and avoid going to the doctor and just get sicker. Costing businesses and taxpayers even more. All while there will be a debate about whether to extend ACA subsidies for 12.5m people. Which is simultaneously the most important and most worthless conversation our politicians will have. It’s important that the money be there for healthcare. It’s ridiculously worthless because that money goes right into the pockets of big insurance companies for plans that have ~5k deductibles and 18k OOP max, which few can afford when something bad happens. But hey. Let’s play gotcha about global politics instead of dealing with something that impacts all Americans and the economy more than any 1 percent interest rate cut ever will.

Urgent cares suck. I work on the opposite end of the body, graduated training almost 10y ago, and know that diarrhea isn’t an indication for a CT abdomen.
It's just unbelievable. Would love to know what percentage of patients in the ER presenting with diarrhea features automatically get a CT now in many centers. It's the chief and basically only complaint of the first CT exams of the night.
