Sex change, for it to occur in humans, would involve a complete transformation of the reproductive system to the opposite type to support the production and release of the other gamete type. But this only occurs in species with high developmental plasticity evolved for this, such as clownfish. Mammals do not have this evolved system nor do we have the anatomical plasticity to artificially institute it. Secondary sex characteristics can be altered through hormones. But those traits do not define sex. Primary sex characteristics affected by cross-sex hormones (at most) experience partial atrophy but no switch to the opposite sex type. For example, your testes or ovaries will not switch to the opposite type. But they will experience partial tissue degeneration, because you are depriving your sex-specific gonads with the hormones they need to stay healthy. Sex change is a process relegated to simpler species with evolved systems for it, not humans.

Typical of your lack of actual expertise. Clinicians don’t claim that transition changes *genotypic* sex (i.e. DNA) but *phenotypic* primary and secondary sex characteristics like endocrine balance and physical anatomy which are very changeable with hormones and/or surgery.

I am posting an open letter to the Prime Minister of the United Kingdom to help a distressed mother in Oxfordshire- Regarding so-called gender affirming care surgeries. From, Dr. Joseph Chrysostom MBBS, MS(Gen Surg), FRCSEd United Kingdom To, The Rt Hon Sir Keir Starmer MP Prime Minister of the United Kingdom Respected Prime Minister, Subject: Deception and Harm in Gender Surgeries – Urgent Need for Government Action-Please save an English mother from intense grief and distress. I am writing to you as a medical professional with more than 35 years of experience to raise the gravest possible alarm about the continuation of so-called “gender-affirming surgeries” in the United Kingdom. These operations are being performed on otherwise healthy bodies, and they are being described to patients in terms that are false, misleading, and unethical. When money is paid from public purse or privately, this needs investigation for serious fraud by the agencies concerned. Today (6 November 2025), I received a deeply distressing phone call from a mother in Oxfordshire. Her only daughter, aged 18, is scheduled to undergo a bilateral mastectomy next week in a private hospital. The operating surgeons claim that the procedure will “masculinize” her chest and describe it as cosmetic in nature. However, it is biologically impossible to “masculinize” or convert a female chest into a male chest, unless the trillions of cells within the tissues—each carrying XX sex chromosomes—could somehow be transformed into XY chromosomes, which no surgeon can achieve. Therefore, the claim of creating a “male chest” through surgery is scientifically false and misleading. I believe this constitutes a serious deception of patients and families and warrants urgent investigation by the General Medical Council (GMC), the Royal Colleges, and other relevant regulatory bodies. In my professional opinion, this operation cannot be regarded as cosmetic. A truly cosmetic procedure should not destroy a vital physiological function of the human body. In this case, the patient will permanently lose her ability to lactate in the event of a future pregnancy — a natural physiological function. Furthermore, she will lose the sexual sensations inherently present in the normal nipples and areolae, which are essential for normal sexual response. On these grounds, this surgery cannot, under any circumstances, be considered cosmetic. It is important to highlight that, according to the Oxford Handbook of Paediatrics, an 18-year-old is still considered an adolescent. At this stage, the brain is still maturing, which raises medical concerns about the individual’s ability to provide fully informed consent. Moreover, in my view, any consent obtained through misleading claims about chest “masculinization” or presented as a form of cosmetic surgery cannot be regarded as valid. The mother is devastated. This is her only child. She fears that this mastectomy will be followed by cross-sex hormone treatments and, eventually, phalloplasty, leading to irreversible sterility and other serious medical complications. Prime Minister, this mother is one of your citizens, and she is pleading for help. These interventions will destroy her legitimate hope of one day holding her own grandchild in her arms. I earnestly urge you to take immediate action to prevent this operation from going ahead, especially given the strong parliamentary majority your government holds. In it, I explain why biological sex is binary in all anisogamous species, defined strictly by the type of gamete (sperm or ova) an organism’s reproductive system has the biological function to produce. The paper also debunks five activist-inspired models that try to redefine sex: 1. Mating-type model – confuses microbial compatibility types with sexes. 2. Karyotype model – mistakes chromosomal variations for additional sexes. 3. Spectrum model – misuses DSDs to claim sex is continuous. 4. Polythetic model – redefines the sexes as “clusters” of traits rather than categories rooted in reproductive function. (this model is self-refuting, read the paper to understand why) 5. Multilevel model – wrongly distributes “sex” across traits (genes, hormones, behavior) instead of gametes. (this model is also self-refuting) Across all organisms with anisogamy, there are only two fundamental strategies based on reproductive function: male and female. Everything else—chromosomes, hormones, morphology—is derivative, not definitional. I'm constantly amazed & confused by the use of ad-hominems—on public platforms—by academics. Especially when it's a philosopher, I try to understand the motivation and provide a bit of feedback via email. It generally fails, as it did here, spectacularly. Today's example is from a UVM philosopher, who completely shut me down, writing only "Please do not contact me again." Below is: 1. His initial Bluesky post 2. My email response 3. His reply 4. His follow-up Bluesky post threatening to share my email if I contacted him again. The only email I sent him is the one below. So I'm heading him off and just revealing my nastiness right here to all on X. Links at the end. 1. Gender identity is an incoherent activist-crafted concept that permits individuals to assert inner identities that contradict observable reality, while framing it as “discrimination” when others refuse to treat that identity claim as objectively true. As a protected characteristic, it collides directly with the sex-based protections of women and girls, and when taught in schools as scientific fact, untethers children from reality and leads those most vulnerable down a path towards irreversible, unproven medical interventions.
